On-Site Medical Clinic Premium Income

Do you know if there is a specific line on the annual statement for premiums received for on-site medical clinics or if they should be reported as a write in? Would these be reported on the Supplemental Health Care Exhibit also?

Page 4 Line 4 "Fee for service (net of $.............. medical expenses)" may be what you are looking for. The NAIC Annual Statement Instructions say this line should include:

Revenue recognized by the reporting entity for provision of health services to non members by reporting entity providers and to members through provision of health services excluded from their prepaid benefit packages. Include in the inside amount, the medical expenses associated with fee-for-service business.
