Does the Company have to note a change in the custodian here from the prior year (changes in 28.01), or is this just to disclose a change in the agreement?

Based on the way the interrogatory is worded, I would say this are looking for changes in the custodians used:

28.03 Have there been any changes, including name changes, in the custodian(s) identified in 28.01 during the current year? Yes [ ] No [ ]
