Page 30 not complete

I am looking at a Company and their page 30 is not complete. Are their exceptions to completing this?

AS instructions say "A schedule must be prepared and submitted to the state of domicile for each jurisdiction in which the company has written direct business, or has direct amounts paid, incurred or unpaid for provisions of health care services." No mention of any exceptions and I am not aware of any.
